Notice to All Physicians and Nursing Staff Treating Me For COVID-19
To whom it may concern:
Enclosed is a Letter of Intent advising you that if being treated for COVID-19, I DO NOT wish to be treated with the drug Remdesivir. Nor do I wish to be intubated or placed on a mechanical ventilation device.
Further, please also find notice that you are hereby instructed to document the above medical provisos in all medical reports, records, and charts generated henceforth during my treatment/stay.
Additionally, in lieu of being treated with Remdesivir, I demand to be treated with the NIH-approved drug Ivermectin, as Ivermectin has been approved by the National Institute for Health under NIH.gov Table 2e. Characteristics of Antiviral Agents That Are Approved or Under Evaluation for COVID-19, as provided: (https://www.covid19treatmentguidelines.nih.gov/tables/table-2e/) last dated July 08, 2021.
As is clearly illustrated by this NIH table/chart, Ivermectin is just as approved and just as evaluated as Remdesivir. (copy enclosed)
For your convenience, I am providing you a hard copy printout of the NIH.gov webpage covering the appropriate use of the drug Ivermectin. Including the dosing regimens, adverse events, monitoring parameters, drug-drug interaction potential, and the links to clinical trials. Correspondingly, I wish for this therapeutic stipulation regarding Ivermectin to be placed, again, in all medical charts, records, and reports for reference and continued implementation by all medical staff at this facility during my time in treatment.

Lastly, you should bear in mind that my life and the life of my loved ones are worth more to each of us than the $2,400.00 to $3,100.00 payment alluded to by the CMS. Including the 20% bonus payout stratagem you are being incentivized with, for the use of Remdesivir on hospital patents over the safer and just as effective drug Ivermectin.
